You will be required to re-enroll at the clinic if you haven't visited the clinic for 3 years. This is a requirement from the Ministry of Health.
If the enrollment has not expired, but about to expire, then you can come in for a doctors visit for a general check-up and update your details
If it has just expired, you can send in the below request first or call us. We will respond back to you, if we need you to fill in a full enrollment and whether you need to send your proof of eligibility details again.
Re-enrollment is a Ministry of Health requirement. Please ensure you update your details within the timeframe to maintain continuous care at our clinic.
